The Chaotic Phantasmal Sacred Beasts
Fusion Effect Monster
Card text
3 Level 10 monsters that cannot be Normal Summoned/Set Must be either Fusion Summoned, or Special Summoned (from your Extra Deck) by sending the above cards you control to the GY. You can only Special Summon "The Chaotic Phantasmal Sacred Beasts" once per turn this way, no matter which method you use. The first two times this card on the field would be destroyed by card effect each turn, it is not destroyed. Once per Chain, up to thrice per turn (Quick Effect): You can target 1 face-up monster your opponent controls; negate its effects (until the end of this turn), then you can gain LP equal to half that monster's ATK.

Explained
A Fusion of 3 Level 10 monsters that cannot be Normal Summoned/Set, or summoned by sending them from your field to the GY. It survives the first two destructions by card effect each turn and can negate an opponent's monster up to 3 times per turn, gaining LP.
In Sacred Beast and Phantasm decks as the boss after the three Sacred Beasts are out.
- Using Tokens or cards in the Spell & Trap Zone for its alternate summon.
- Reviving it before it was properly summoned once.
How it works
Materials: 3 Level 10 monsters that cannot be Normal Summoned/Set. Must be Fusion Summoned, or Special Summoned from your Extra Deck by sending those cards you control to the GY. Once per turn, either way.
- Continuous: The first two times this card on the field would be destroyed by a card effect each turn, it isn’t destroyed.
- Quick Effect (once per Chain, up to 3 times per turn): Target 1 face-up monster your opponent controls; negate its effects until the end of the turn, then you can gain LP equal to half its ATK. Not in the Damage Step.
Rulings in plain English
Alternate summon: Not an effect, so no chain. It’s not a Fusion Summon, but the monsters are sent as Fusion Material. You can use face-down monsters in your Monster Zone, but not Tokens or cards in the Spell & Trap Zone. Sending and summoning aren’t simultaneous.
Revival: Both methods are proper summons. Other cards can’t summon it unless they ignore summoning conditions, and from the GY or banishment it must have been properly summoned first.
Negate: Negate first; only if that worked can you gain LP.
It’s a “Phantasm” monster.
